Under the Durable Medical Equipment your Medical Insurance will cover medically necessary CGM/ Back, Shoulder, Neck, Wrist, Knee & Ankle, Braces or Supports.

Do I have to pay for Shipping?
No, At Pharm Source Inc we do not believe in charging our customers on shipping! Your Braces / Devices are Ships for free by FedEx Priority will arrive at your home in 2-3 days.

Are there any forms I must send back to you once I receive my DME Equipment?
Yes, Once you receive your brace you must sign and send back to us the Assignment of Benefits (AOB) sheet. You can either mail back to us in a prepaid envelope or fax the same at 808-400-6066.

I have Medicare is there any out of Pocket Expenses?
If Medicare is your Primary Insurance and you are looking to get a Brace or Support covered, you may be responsible for the 20% of the cost under Medicare’s 80/20. If you have a Secondary Insurance then they would cover the 20%.

I have Medicare and I can't affort to pay the 20%?
If you do not have a Medicare supplement policy and are unable to afford your 20% Co-Insurance there is a hardship letter that you will need to sign stating you are unable to pay the 20%.

Does Insurance cover DME/CGM participate with my Insurance Company, Medicare or Medicaid?
Our Parent Company, Metro Drugs, LLC, has over 25 years of experience in billing insurance companies for Durable Medical Equipment products including DME/CGM. Pharm Source is contracted with all of the major insurance companies, Medicare, and most Medicaid plans.
